Hardware Request Forms


Data Center Hardware Installation Request

  • This form is to be used by the Minnesota State Colleges and Universities internal and external customers to request hardware installation at any of the ITS administrative computer centers located throughout the State of Minnesota.
  • Please submit one set of Installation Request forms for each server/system/application (including hardware enhancements) requested to the Director, IT Infrastructure, located at Office of the Chancellor, Wells Fargo Place, 30 7th Street E., Suite 350, Saint Paul Minnesota 55101-7804 (651) 201-1412.
  • Pages 4 thru 6, (Installation Requirements, Electrical & Environmental Information, & Cabling/Connectivity Requirements), must be reiterated (copied) for each physical device associated with this request.
  • * Denotes Required Fields.
  • MnSCU Infrastructure Support (Operations & Network) and Security Staff review this information to assess installation requirements.
  • This information is used to develop a list of facility and network equipment needed for this installation.
  • A project timeline will be created and forwarded to MnSCU ITS Management for approval.
  • All installations must have Management approval prior to installation.
  • This process takes 4 to 6 weeks. Missing information on the form will likely delay this process.

Data Center Hardware Decommission Request

  • This form is to be used by the Minnesota State Colleges and Universities internal and external customers to request the decommission of hardware at any of the ITS administrative computer centers located throughout the State of Minnesota.
  • Please submit a Decommission Request form for each server/system/application requested to the Director, IT Infrastructure, located at Office of the Chancellor, Wells Fargo Place, 30 7th Street E., Suite 350, Saint Paul Minnesota 55101-7804 (651) 201-1412.
  • * Denotes Required Fields.
  • MnSCU Infrastructure Support (Operations & Network) and Security Staff review this information to assess decommission requirements.
  • All decommissions must have Management approval prior to decommission taking place.
  • This process takes 4 to 6 weeks. Missing information on the form will likely delay this process.
