  Submit completed form with attached copies of applicable Travel Warnings/ Alerts to the college or university president or chancellor. This form should be retained by the college, university, or system office.
  
  

• Employee Acknowledgment of International Travel Risks and Responsibilities - Word document developed by the Office of General Counsel, this form is required as a condition of approval of travel to countries under a travel warning as part of the procedure under System Procedure 5.19.3 Part 10.  Provide the original of this form to college/university Human Resources Office; copies are to be provided to the employee and designated administrator.
